Fatty liver is frequently silent. When symptoms do appear they tend to be non-specific — fatigue, a dull ache or fullness under the right ribs, unexplained weight change — which is why diagnosis usually rests on tests rather than on how you feel.

Diagnosis normally combines liver blood tests (ALT, AST, GGT, bilirubin), imaging such as ultrasound, elastography or MRI, and a scoring tool that estimates the chance of significant scarring.
